How to report Medical ID Theft or Fraud.

HealthLock makes it easy to report suspected medical identity theft or fraud so we can begin investigating and protecting your medical records right away. You can report potential issues in two ways:

  1. Unknown Provider
    If you see a provider in your dashboard that you don’t recognize, click “I DON’T RECOGNIZE THIS PROVIDER.” We’ll investigate the issue on your behalf and work to restore accurate medical records.

  2. Unfamiliar Claim
    If a claim appears that you don’t recognize, click “REPORT POTENTIAL FRAUD.”
    Our team will determine whether it’s a billing error or true fraud and take the appropriate steps to help safeguard your information and restore your privacy.

Most HealthLock plans include medical fraud detection and resolution at no additional cost.
