There are two different ways you can report medical identity theft or fraud to us:
- If an unknown provider appears in your dashboard, click the 'I DON'T RECOGNIZE THIS PROVIDER' button. We'll work to investigate on your behalf and restore accurate medical records.
- If a claim appears that you don't recognize, click the 'REPORT POTENTIAL FRAUD' button. Again, we will start working on your behalf to uncover whether this is an error or actual fraud and take the appropriate steps to help restore your privacy.
On most plans, these services are included at no additional cost.
