You can typically find your Subscriber ID number on the front of your health insurance card. Depending on your insurance carrier, this number may also be called your Member ID or Medical Record Number (MRN).
Your Group Number—sometimes referred to as your Policy Holder ID—is also listed on your insurance card. Both numbers help identify you and your coverage when submitting claims or connecting your insurance to HealthLock.
Keep in mind that insurance cards may vary depending on your carrier and the type of plan you have.
Example: On a BlueCross BlueShield card, your Subscriber (or Member) ID and Group Number are displayed on the front of the card.
