Your Insurance ID # (also known as your Subscriber ID #) is needed to verify your account. Depending on your insurance provider, you can find your Subscriber ID on the front of your medical insurance card.
Your Subscriber ID may also be referred to as your Member ID or Medical Record Number (MRN). Your Group Number may also appear and is sometimes called your Policyholder ID (see BlueCross Blue Shield sample card below).
Please note that insurance cards may vary depending on the insurance company and type of plan you have. For verification and claim management, your card should be uploaded to your HealthLock Member Portal.

To Upload Your Insurance Card:
Log in to your HealthLock Member Portal.
From the Dashboard, select Settings to expand options, then choose Documents.
In the upper-right corner, select the Upload button.
When the Upload Documents box appears, select Choose a file to upload.
Select a Category from the list: Insurance Card, Invoice, Receipt, or Other.
Add a brief Description of the document you’re uploading, then select UPLOAD.
In the Documents view, you will see your uploaded insurance card along with any signed HIPAA Authorization Forms or other documents related to your insurance and claims.
