Our auditors work on your behalf to help ensure your medical bills and claims are accurate. To do this, they may need access to your Personal Health Information (PHI) from your healthcare providers, insurance companies, or other related entities.


Many of these organizations are considered “Covered Entities” under the Health Insurance Portability and Accountability Act (HIPAA) and are legally required to obtain your authorization before sharing your medical information.


By signing both the HIPAA Authorization Form and the HealthLock Provider Authorization Form, you grant HealthLock permission to securely access your medical and billing information so your auditor can begin investigating your claims and resolving any potential errors.
